In order to credit your account accurately,  it is very important that you provide us with the

patient name and account number found on your billing statement.

Refund Policy:

All payments on-line are for goods and patient services dispensed or performed  in the office, hospital or surgery center.

If a payment refund is requested due to incorrect billing using commercial insurance, a copy of the Explanation of Benefits (EOB) from

the insurance carrier is necessary for verification.  No refunds for Cash-Pay medical services which are due at the time of service.
